DRIVER LICENSE PROCEDURES

Procedures Guide

Change of Address

  1. For Notification Purposes only
    1. Must file within 30 days of change
    2. Upon receipt of your notification, only your driver record will be updated
    3. Mail your notification with your signature to: P.O. Box 30340, Honolulu, HI
      96820-0340 or fax to 808-832-2904
  2. For issuance of a new license with your updated address
    1. Report to Driver License Location for Application
    2. Fingerprinting
    3. Payment of Fees and Photographing

Duplicate By Mail

  1. Send Letter Requesting for a Duplicate of your Hawaii Driver License
  2. Include Your Name, Social Security Number or Hawaii Driver License Number, Date of Birth, Email address (optional), reason for the request, Signature and the fee of $6.00 (certified check or money order payable to City & County of Honolulu).
  3. Mail Request to:           Driver License Section

                                        Honolulu, HI 96820-0340
    Or Fax to:                    (808) 832-2904
  4. If there is no compliance requirement pending in any jurisdiction (including this state) that prevents you from getting a license, we will mail to the address on file a valid-without-photo license.
  5. If your image is found in our driver database, both the photo and the signature will be utilized. 

Instruction Permits and New Licenses:

  1. Minors obtain Parental Consent Form. If both parents are not able to accompany you to sign the parental consent form and unable to sign and have notarized the reverse side of the application form, the absent parent(s) may sign and have notarized a separate Parental Consent Affidavit form: http://www1.honolulu.gov/csd/csldl239.pdf
  2. If not previously submitted on or after March 5, 2012, documentary proof of legal name, date of birth, social security number and legal presence. Acceptable documents may be found at:  http://www1.honolulu.gov/csd/vehicle/dlrequirements.htm.
  3. Submit to Written Exam, Eye Test and Fingerprinting.
  4. Payment of Fees and Photographing.
  5. Road Test.

New Licenses - Applicants with Valid U.S. License:

  1. Submit documentary proof of legal name, date of birth, legal presence, social security number and two forms of proof of principal residence in Hawaii. Acceptable documents may be found at:  http://www1.honolulu.gov/csd/vehicle/dlrequirements.htm.
  2. Submit to Written Exam, Eye Test and Fingerprinting.
  3. Payment of Fees and Photographing.
  4. Road Test (may be waived).
  5. Must be 18 years of age or older.

Renewal By Mail

  1. Send Letter Requesting to Renew Hawaii Driver License.
  2. Include Your Name, Social Security Number or Hawaii Driver License Number, Date of Birth, Email address (optional) and Signature.
  3. Mail Request to:      Driver License Section

    Or Fax to:               (808) 832-2904
  4. When we receive your request, we will mail a renewal packet to you.
  5. You must complete these forms (included in the packet) and send back with payment:
    1. Medical Report Form - examination must be completed and signed by a licensed physician (MD or DO) within 6 months.
    2. Certificate of Eye Examination - examination must be completed and signed by a licensed physician within 6 months.
    3. Statement of Driver's License Possession - must be completed and notarized.
  6. If not previously submitted on or after May 1, 2014, documentary proof of legal name, date of birth, legal presence, social security number and two forms of proof of principal residence. Acceptable documents may be found at:  http://www1.honolulu.gov/csd/vehicle/dlrequirements.htm.
  7. Full instructions will be included in the Renewal packet.
  8. If there is no compliance requirement pending in any jurisdiction (including this state) that prevents you from getting a license, we will mail you a valid-without-photo license.  If your image is found in our driver database, both the photo and the signature will be utilized

Renewal of Valid Hawaii Driver's License:

  1. Report to Driver License Location for Application.
  2. If not previously submitted on or after May 1, 2014, documentary proof of legal name, date of birth, legal presence, social security number and two forms of proof of principal residence in Hawaii.  Acceptable documents may be found at:  http://www1.honolulu.gov/csd/vehicle/dlrequirements.htm.
  3. Eye Test and Fingerprinting.
  4. Payment of Fees and Photographing.

Replacements and Duplicates:

  1. Report to Driver License Location for Application.
  2. Eye Test and Fingerprinting.
  3. Payment of Fees and Photographing.

Replacements and Duplicates - Name change:

  1. Report to Driver License Location for Application.
  2. Submit proof of name change. Acceptable documents may be found at:  http://www1.honolulu.gov/csd/vehicle/dlrequirements.htm.
  3. Eye Test and Fingerprinting.
  4. Payment of Fees and Photographing.

Traffic Violations Bureau Clearance/Record

  1. Please contact the Traffic Violations Bureau at (808) 538-5500 for driving record or clearance.

Verification Letter

  1. Send letter requesting verification of your Hawaii Driver License with Your Name, Address, Social  Security Number, Date of Birth, and Signature.

Military Extension

  1. For Military Extension verification letter, you must submit a copy of your deployment orders to show the date of return to the U.S., duration and place of deployment.
