**Pothole Service Request Form and Phone Line**

**If the Pothole is on a State Road or Highway (listed below) please call the State Pothole Report Phone Line at 536-7852.**
Kalanianaole Hwy.	Likelike Hwy.	(most of) Kamehameha Hwy.
Kahekili Hwy.	Nimitz Hwy.	(most of) Farrington Hwy.
H1, H2 or H3 (including on & off ramps)	Ala Moana Blvd.
Pali Hwy.	Vineyard Blvd.

To report a City Pothole that needs repair click on Pothole Request Form and fill in the requested information. When completed click on the Submit Form button at the bottom of the form to send it to the City and County of Honolulu's Division of Road Maintenance.

You may be contacted for more information about the location of the Pothole if necessary.
