Person with a Disability Bus PassInformationWho Qualifies?Any person with a physical or mental impairment that substantially limits one or more of the major life activities as outlined below and due to such impairment is unable to do one or more of the following functions necessary to effectively use the City bus system's facilities without significant difficulty:

Hours for obtaining approvals are between 7:45 a.m. and 3:45 p.m., Monday through Fridays, except Holidays. The application is provided here in Adobe Acrobat Portable Document Format (PDF) and requires the Acrobat Reader freeware program to view, fill in, and print. Application forms are also available at all Satellite City Halls, TheBus pass office at 811 Middle Street, most major hospitals' social service departments, or by calling (808) 848-4444. Completed application forms should be mailed or delivered to: TheBus Pass Office 811 Middle Street Honolulu, Hawaii 96819 848-4444 TTY 848-4500 After approval, you may purchase your pass. COSTIf your disability has been certified by a licensed medical physician as being permanent, or you present a MEDICARE card you will have the choice of a one-year bus pass for unlimited rides for a cost of $30.00 or you may use your Medicare card or if you don't have one purchase a four-year Reduced Fare ID card for $10.00 and ride TheBus for one-half the adult fare or purchase a monthly pass for $5.00 each month.
