DOCUMENTS REQUIRED:

     ** Only original documents accepted.  No faxes or copies. **

Rule3-82-38.5(c) Registration of Employees Persons who apply to the Commission for registration shall present:

1. Positive identification in the form of either a passport, a laminated driver's license with photograph, or other laminated government identification with a photograph (i.e. Military ID);
2. Proof of Social Security number;
3. A current satisfactory tuberculosis (TB) clearance report [SUSPENDED UNTIL FURTHER NOTICE]

Also REQUIRED:

• MANAGERS ONLY: A signed letter from employer, authorizing registration as a Manager.
• $10.00 [CASH ONLY] processing fee for each establishment you are registering to be employed at.
