What to bring to register for a Liquor Commission card:
      **
Only original documents accepted.  No faxes or copies. **

  1. TB clearance - From any authorized medical facility, to include military, personal physician or other similar facility, verifying a 0-9 mm negative reading, or X-ray clearance. 
    -- A TB clearance is not required for Retail License employees. --
  2. A current government issued card with photo, Military ID card, Passport.
  3. Proof of Social Security number.
  4. MANAGERS ONLY: A signed letter from employer, authorizing registration as a Manager.
  5. $10.00 CASH* for each establishment you are registering to be employed at.

    (*This processing fee, for each Liquor Commission registration card, is - CASH ONLY )
