AMERICANS WITH DISABILITIES ACT (ADA) CURB RAMP REQUEST FORM AVAILABLE

            Mobility-impaired persons wishing to request curb ramps improvements on City streets to access specific destinations may now download a request form from the City’s website.  The web address is http://www.co.honolulu.hi.us/parks/civil/index1.htm.

             To initiate a request, qualified persons must submit a completed form by hand, mail, or fax with their name and phone number.  Requestors needing help to complete or submit a form may call 523-4672.
