Requests for Proposals for Home and Community Based Services
The Elderly Affairs Division of the Department of Community Services is accepting proposals from agencies and organizations interested in providing home and community based services to eligible older adults on
Funding is available for the provision of the following services to older adults age 60 and older using Federal and/or State funding: Attendant Care, Case Management with Supplemental Services (Chore and Homemaker), Case Management for Abused Elders, Group Dining, Health Maintenance, Home-Delivered Meals, Housing Assistance, Legal, Para-Professional, Personal Care, Respite Linkage,
Proposals will also be accepted for the provision of the following services to qualified unpaid caregivers of any age who provide assistance to older adults under the National Family Caregiver Support Program using Federal funding: Access Assistance (Case Management and Supplemental Services), Caregiver Respite, and Caregiver Support (Group 1: Counseling, Support Groups, Training, and Group 2: Information Services).
For-profit as well as non-profit organizations are welcome to submit proposals. Non-profits applicants must be tax-exempt, charitable 501 (c) (3) organizations as defined by the Internal Revenue Service.
